Minnesota Survey of Social/Emotional Supports and Services

Thank you for participating in this survey and helping to gather valuable information on the social/emotional supports and services currently offered in Minnesota schools. Please follow the directions below to complete the survey for your school (i.e., the school to which your introductory letter was sent).

We are no longer collecting surveys. Thank you for your participation!

1. Assemble a voluntary team of school personnel.
Ideally, teams will include: student support personnel (school social worker, school nurse, school counselor, chemical health staff, behavior specialist), administrator, general and special education teachers. It may be helpful to use a team already existing in the school, such as your problem-solving or child study team.

2. CLICK HERE to access the survey. It may be helpful to print a copy of the survey to share with your team before filling it out together online. CLICK HERE to download a PDF copy of the survey.

3. Complete the survey collaboratively.
If you complete the survey in multiple sessions, simply close the browser and (as long as you use the same computer) you will automatically return to that page of the survey. The survey will take approximately 30-45 minutes to complete.
